Important Health Coverage Tax Documents

If you were eligible for Health Insurance during 2025, 1095-C, Employer Provided Health Insurance Offer and Coverage forms are now available.   

If you are a current employee of ISD 345, you may electronically download your form on ESS.  Open ESS and navigate to My Payroll > ACA Form 1095. Choose 2025 and print the form.  

If you are no longer an employee you may request a copy of the form by contacting the District by one of the methods below:

Ask your tax preparer if you will need a 1095 form, not all require it.
