Flu Shot Clinic Available for Students and Staff

September 4th, 2020

Dear New London-Spicer Parents/Guardians:

Carris Health is working with New London-Spicer Schools to give the seasonal influenza vaccine to students in grades K-12. This vaccine will protect against the influenza strains that are expected to circulate this year. We recognize that preventing influenza and other respiratory illnesses to keep students, staff, families, and communities healthy is more important this year than ever before. We will be holding a flu vaccination clinic on Wednesday, September 30th at NLS Schools.

If you would like your child to receive the influenza vaccine (shot or mist) you will need to read and fill out the Influenza Vaccination Clinic Form and sign the Admission Consent. You can print these forms on your own or request a copy from the health office, 320.354.2252 ext. 2612. Please complete and return the consent forms to your child's school by Wednesday, September 23rd, 2020. These forms are located on the school's homepage under announcements.

The Admission Consent is giving consent for your child to receive the vaccine and for us to bill your insurance. Completing these forms will allow us to give your child the vaccine. We will file charges for the vaccine with your insurance provider. Any unpaid balance will be your responsibility. The elementary school will have vaccinations completed in the morning and at the middle/high schools in the afternoon. For students who are in grades 7-12 doing Hybrid Learning and would like to receive the flu vaccine, complete and turn in the required forms by 9/23, then come into school in the afternoon on 9/30 to receive vaccine. Staff will share a more specific times closer to the actual date.

Please read the Vaccine Information Sheets (Inactivated and Live) about the disease and the vaccine. If at any time you change your mind about having your child vaccinated, or have questions about the flu vaccine, please contact your child's clinic. You can also visit our website at www.carrishealth.com Your child's health care provider at your clinic can also give your child the seasonal influenza vaccine, as well as answer your questions about the influenza virus or vaccine. Visit the CDC or MDH influenza website for more information: www.cdc.gov/flu OR https://www.health.state.mn.us/diseases/flu/basics/fluparent.html
